1. The Consent for Treatment
2. Dr. Flint's education and work experience
3. Procedures to comply with the Personal Information Protection Act, Canada.

Before telephone therapy is started, this form must be completed and faxed to Dr. Flint. Please telephone to discuss the requirements for treatment, such as:
1. Treatment history and general treatment issues.
2. Current Therapist or support network.
3. Fee for service.
